What happens with a genuine emergency at 3am?

It follows the escalation path you define — typically an immediate call or message to your named on-call contact, with the criteria for waking someone agreed in writing beforehand.

Can we start with after-hours only?

Yes, and it is a sensible first step. Two agents covering evenings is a low-risk way to see how the arrangement works before extending it.
